There is no difference the two. Health insurance policy are generally called as mediclaim policies as they reimburse the hospitalization expenses . There are some plans which payout pre defined benefit irrespective of the hospitalization expenses. These plans could be offering a combination of daily hospitalization cash benefit, specified surgeries benefit and critical illness benefit. However, such plan are issued by life insurance companies.

Health Insurance & Mediclaim are generally refer to the same. But with new health plans in market, it's better to differentiate the two. Mediclaim is insurance where the insured person is reimbursed/cashless claim when he/she is hospitazed. On the other hand, a health insurance is a more generic term. It can refer to mediclaim, critical illness, plans for medical expenses(e.g. LIC health) etc.
